Instructions:

Click on service area to retrieve those forms needed.

Printing and submitting NCR forms is only to be used as backup when no forms are available from the site. NCR Forms will have NCR in the file name or in the footer of the form — these forms will need to be printed in triplicate: one for Central, one for the Site and one for the Parent.

To print the Adobe Acrobat files, one must have Adobe Acrobat Reader installed; available for free at http://www.adobe.com.

All HSCDP computers have Adobe Acrobat Reader installed.
To print an Adobe Acrobat file (.pdf) use the print icon embedded in the Adobe Acrobat Reader Plug-in and not the Internet Explorer print icon.

Performance Review Form Instructions:

  • Open "P401_Perfomance_Review" located under the "Personnel" link below
  • Save as "Performance Review_Person's_Name_and_Date" e.g. "Perfomance_Review_Jack_Jones_02-10-05"
    in any destination folder on your hard drive
  • Click "Tools" on MSWord Toolbar
  • Click "Protect Document"
  • Toggle the "Forms" radio button
  • Click "OK"
  • Fill in Form
  • Save
